AIM Palliative Home Health Services offers a patient referral and information line.  Patients or families may phone in to ask questions about AIM Palliative Home Health.  Physicians and staff may also use the AIM phone line to call in referrals or get information.  The AIM fax number is provided to allow transmission of information.  If you prefer to call Stat Home Health with the referral, please identify this patient as an AIM patient.

Physicians may also wish to have the patient followed by the AIM Medical Director.  This is often a benefit to physicians in a large group where partners may not be familiar enough with the patient to manage their pain/symptoms at home.  The AIM Team has the benefit of a Board Certified Hospice and Palliative Care Physician to call as back-up if the Primary Care Physician is not available.
